Adult Plan

$385/yr. 14 Years Old and Older
Plan includes:

- **If you get diagnosed with periodontal disease after your membership starts, you will be required to upgrade to the periodontal plan. You will be required to pay an additional $110.

Periodontal Plan

$495/yr. Anyone Diagnosed with Periodontal Disease 14 Years old and Older
Plan includes:
- 2 FREE periodontal maintenances
- 2 FREE check-up exams
- FREE intra-oral x-rays when needed
- $50 off total price of periodontal topical antibiotics
- 15% off treatment fees*
- 10% off in oral care products
- $500 off orthodontic straight-wire brackets (one time use)
- $250 off orthodontic clear aligners (one time use)

Important Notes

NBF Dental Plan is NOT INSURANCE and is not to be combined with dental insurance.
The sign-up fee is non-refundable. If you get insurance during the year, you will use the fees from your insurance company with no funds returned to you.
The yearly sign-up fee will AUTOMATICALLY be renewed each year from the date of your sign-up. You need to leave a credit card, no debit, on file for the renewal. If the transaction is declined, there will be an automatic $50 fee applied. Your credit card will not be run any other time without permission.
If you choose to NOT renew this plan, you are to contact our office 30 calendar days before your automatic renewal date.
